Administrator access authorisation form

At Resolve we take security very seriously. We work responsibly for our clients to ensure that the security of their systems is maintained. Administrative access is the highest level of access to your system and has risks associated with it, which is why we require this form to be completed before releasing these details.

Once Resolve has received a completed copy of this form, the access details for the administrative account will be sent to the email address specified on the form.

You can either complete the form below, or download a copy and email it to support@resolve.co.uk This form must be signed by a director or partner of your organisation.

Once the form is filled out, a verification email will be sent to the email address you entered when signing the form. If this step isn’t completed, Resolve won’t receive the form.
